Payment and Insurance

INSURANCE. I am a fee-for-service provider.  I do not accept Medicare or any other insurance program.

PAYMENT. Payment is due on the day of the service unless other arrangements have been made in advance. A receipt with codes for insurance or taxes is given at the time of payment. If you have out-of-network partial repayment benefits, this receipt may help you recover those benefits.
